Welcome to the Laser Welding Course Indemnity Form!

We are excited to have you join us. Safety is our absolute priority in all aspects of this training.

To ensure we adhere to regulatory safety compliance and that you fully understand these risks before engaging in high-power practical work, all students are required to sign a formal Student Indemnity & Waiver of Liability Agreement.

This form must be completed, signed, witnessed, and returned to us before you can receive access to the official course or physical training workshop.

3. Read Thoroughly: Please take the time to read the entire agreement carefully. It is a legally binding document that affects your legal rights.
4. Execute: Complete the Student (Indemnifier) section.
5. You must sign and date the form.
6. You must have a witness sign the witness declaration section on the same day.
7. Return: Scan the fully signed document and email it to us at info@ambitious.co.za with the subject line “Completed Indemnity Form – Your Full Name“.

Note: Indemnity Agreement Form will be reviewed by administration.

The process could take up to 7 business days.

Once the form has been approved, the Official Laser Welding Course will become available.
